Continuing Competency Reporting Form

According to the Health Professions Act, the Denturists Profession Regulation, the College’s Bylaws and the Continuing Competency Rules, all Regulated Members are required to complete and submit an annual report of their continuing competency activities.

Members can submit a Continuing Competency Reporting Form when they have completed a course throughout the calendar year if they so desire, however, the annual reporting form which is issued with renewal documents, must be completed and returned with the renewal application documents on or before November 1st of each year.

To submit at other times, please access the appropriate form below, complete and submit to the College with substantiating documentation for the submitted course(s).
